Welcome to our office and thank you for choosing Great Lakes Orthopedics & Sports Medicine, P.C. as your health care provider. We are committed to your treatment being successful. Please understand that payment of your bill is considered a part of your treatment.

 

Regarding Insurance

We may accept assignment of insurance benefits. It is your responsibility to verify whether Great Lakes Orthopedics & Sports Medicine, P.C. is a participating provider with your plan. In accordance with your insurance provider co-pays and deductibles are to be paid prior to service.

Your insurance policy is a contract between you and your insurance company. You are responsible for the balance whether your insurance pays or not. We cannot bill your insurance company unless you provide us your correct insurance information. Please be aware that some, and perhaps all, of the services provided may be non-covered services under your medical insurance. A copy of your insurance card will be made at your first visit and must be updated should your insurance change. It is your responsibility to notify the office of any insurance coverage changes prior to services for us to serve you best.

 

Usual and Customary Rates

Our practice is committed to providing the best treatment for our patients. You are responsible for payment regardless of an insurance company’s arbitrary determination of usual and customary rates.

 

Self Pay

Great Lakes Orthopedics & Sports Medicine, P.C. does have options available for patients who do not have health insurance coverage to assist patients in reducing their healthcare expenses. If you do not have health insurance coverage please notify the staff when your appointment is made and/or ask to speak with one of our billing representatives

 

Pre-Certification

Many insurance companies may require that you obtain pre-authorization from your Primary Care Physician and/or from your carrier prior to services being rendered. Our staff is available to assist you in understanding how to obtain the necessary authorizations. However, it is the responsibility of the patient to make sure prior authorization is obtained and the patient is financially responsible if the pre-certification is not obtained.

 

Minor Patients

The parent(s) or guardians of the minor are responsible for full payment. For unaccompanied minors, non-emergency treatment will be denied. A minor, anyone under the age of 18 years and non-emancipated, must be accompanied by an adult 18years or older to authorize treatment prior to receiving health care services. ( IC 16-18-2-5. P. L. 1993, Sec. 1.)

 

Missed Appointment

Please give at least 24 hours in advance when you are unable to make your appointment. Please help us serve you better by keeping scheduled appointments.

 

Payment Plans

If you do not have 100% coverage our billing staff will notify you prior to any procedures and/or surgery. It is your responsibility to settle all of your outstanding balances within 30 days of payment from your insurance company. If you cannot afford the out of pocket expenses we have partnered with CareCredit for all of our payment plans.
