An event/accident in which an employee was injured while executing the scope of their responsibilities during scheduled worked hours while in the routine course of employment.

Process:

  • The employee must complete and submit a first report of injury to their employer, as per your company’s guidelines.
  • The completed first report of injury is sent to the insurance carrier by the employer
  • The determination of the compensatory qualification is verified by the insurance carrier.
  • The insurance carrier may direct care to certain providers of services or providers within their network. Workers’ Compensation laws vary from state to state including Indiana, Illinois and Michigan.
  • Authorization/treatment approval will be provided at the direction of the insurance carrier. This information will be communicated to the patient in a timely manner.

Scheduling:

  • Prior to the scheduling of a workers’ compensation patient, authorization must be obtained from the insurance carrier. Moreover, the authorization is best received from the carrier rather than the employer.
  • The insurance carrier authorization assures Great Lakes Orthopedics & Sports Medicine, P.C. that the policy premiums are current, validity of a policy and the compensability of the claim.
  • The authorization must be in writing and contain 1) contact name, i.e. case manager, adjustor, 2) telephone and fax number of the contact name, 3) claim number and date of injury, 4) job description (if available) and 5) light duty availability.
  • The coordinator for the Workers’ Compensation Department will schedule the patient for their appointment depending on physician availability, injury and urgency of the injured worker.
